- ③ Business registration (copy)
- ④ Proof of identification for manager who will be overseeing passes (proof of employment or 4 social insurance certifications)
- * • Small business owners who are registered as individual companies can use personal ID documents
- ⑤ Agreement of collection and use of personal information by 3rd party provider [→ required at the time of application]
ID and password are provided (via SMS to the phone number of the manager who submitted the application)
